Job description

A Sydney home care provider needs a Care Partner / Case Manager to coordinate support for older clients.

The key facts come first. This is a full-time role in Sydney paying $80,000 - $95,000 per year. You manage individual support plans and lead care coordination for people living at home under the Support at Home program. The team is small and flat, so you get plenty of independence. Visa sponsorship is available for the right candidates.

Your week involves meeting clients, assessing needs, setting up services and keeping each person's supports on track so help arrives when it is needed.

Key responsibilities

  • Build respectful relationships with clients and their families
  • Work closely with support workers and outside providers
  • Set up tailored services that match client goals and health conditions
  • Run assessments, write care plans and review them
  • Decide with client safety, wellbeing and quality of life first
  • Lead and supervise Support Workers to keep service quality and continuity high
  • Track and manage client budgets
  • Follow legislation, funding guidelines and internal policies
  • Keep progress notes accurate and timely; check notes from support workers, suppliers and allied health staff
  • Onboard new clients through intake assessments and meet and greets

Requirements

  • Hands-on aged care, home care, community services or health setting experience (proven)
  • Sound assessment and care planning skills
  • Sound case management skills
  • Confident working alone and handling competing priorities
  • Excellent communication, relationship-building and leadership skills
  • Working knowledge of My Aged Care plus the Aged Care Quality Standards
  • Driver's licence, own vehicle and willingness to travel across Sydney
  • Speaking more than a single language (bilingual or multilingual) is a strong advantage
  • Clinical background such as nursing or allied health: nice to have, not required
